I went mountain bike riding with a mate riding his brad new $4,000 electric bike that we had just collected from the shop. It came with a signed assembly checklist with an additional 'checked by' signature beside the technicians signature who had assembled it... what could go wrong?
We rode around for 10mins or so on flat trails before coming to a steepish incline, whereupon my buddy stood on the pedals for the ascent, when all of a sudden the right pedal came off and he performed a perfect A over C major! The Vienna Boys Choir would have been proud of him after his groin hit the crossbar!!
It turned out that the pedal had unscrewed itself, so it clearly hadn't been tightened, there was no locktite applied, and it hadn't been checked or doubled checked. The signed assembly check sheet was just a red herring... Fortunately, my Buddy didn't want any more children...
